OUR OUTREACH PROGRAMME

The Inkunzi Isematholeni Foundation's harm reduction program represents a crucial initiative aimed at addressing the needs of people who inject drugs (PWID) in the uMgungundlovu District. By providing essential services such as needle exchange, harm reduction education, and advocacy for human rights, the program plays a vital role in reducing the health risks associated with drug use and promoting the dignity and rights of marginalized populations.The program is aligned with the National Drug Master Plan and the National Strategic Plan of 2023 to 2028, demonstrating a commitment to a comprehensive and strategic approach to drug-related issues in South Africa. With outreach-based implementation and a peer-led modality, the initiative is designed to be accessible and responsive to the specific needs of the communities it serves, fostering trust and engagement among participants. By focusing on evidence-based practices and community involvement, the Inkunzi Isematholeni Foundation’s harm reduction program not only directly addresses the health and social challenges faced by PWID but also contributes to broader public health goals and the promotion of human rights.

Key Compnents

Needle Exchange: Facilitating safe and sterile supplies to PWID to reduce the transmission of bloodborne diseases such as HIV and hepatitis.

Harm Reduction Education: Offering training and information to empower individuals with knowledge about safer drug-use practices, overdose prevention, and access to healthcare services.
Advocacy for Human Rights: Ensuring that the rights of PWID are recognized and respected, advocating for policies that promote their health and well-being without stigma or discrimination.
Clinical Services:Providing access to healthcare services tailored to the needs of PWID, including testing and treatment for infectious diseases, mental health support, and substance use disorder treatment.
Psychosocial Support:Offering counseling and support services to help individuals navigate the challenges associated with drug use, promoting overall well-being and facilitating recovery.

HOME AWAY FROM HOME

Our OST Programme

Objective: To reduce opioid addiction rates within the community. To support individuals in their journey towards recovery and reintegration. Treatment Approach: Methadone Treatment: Administering Methadone in a structured manner, ensuring that patients receive their doses under close supervision to minimize misuse and maximize therapeutic effects. Daily Observed Therapy (DOT): This methodology ensures adherence to the treatment plan, as patients take their medication in the presence of a healthcare professional, helping to build a trusting relationship and providing an opportunity for direct support. Initially based at the Drop-In Centre in Pietermaritzburg, the program has also been extended to Richmond, indicating its commitment to reaching a wider audience and addressing the needs of more individuals battling opioid addiction. The program encourages participants to become voices of change within their communities, helping to combat the stigma surrounding addiction and promoting awareness of the resources available for those in need.

Community Re-intergration

The program places a significant emphasis on helping individuals reintegrate into their communities. This involves: Providing resources for employment and education opportunities. Encouraging involvement in community activities that promote positive social interactions and support networks. Empowering individuals to become advocates for change in the fight against substance abuse.

Psychosocial Support

Intensive psychological and social support services, aimed at addressing the underlying issues related to addiction. This may include: Counseling and therapy sessions to work through emotional and psychological challenges. Life skills training to improve coping mechanisms and enhance quality of life. Support groups to foster community and connection among individuals facing similar challenges.

Health Services

Case Finding (HTS). We ensure provision HIV Testing Services to all our enrolled beneficiaries. Any reactive client is initiated on-site by our NIMART Nurses. Non-Reactive Clients are initiated on PrEP and are monitored on our case management tool to ensure less cycle off clients. We also do indexing (Smoke buddy system). Client Retesting is prioritised usually sensitising them about HIVSS and using it for our re-test. TB, Hypertension, Diabetes, STI Screening Adherence Groups. Sexual Reproductive Health Breast Examination. Pap Smear. Family Planning Pregnancy Test Contraceptive Viral Load Case Management Client ART retention is monitored and adherence plans are drafted and signed by beneficiaries. We track 3 Months VL due, 10 Months VL due, Annual VL Due. Track and Tracing We assist our partner facilities with tracking and tracing of clients believed to be part of our PWID database. Hepatitis B Virus We do Hep B Virus testing (OST Clients). Hep B Virus Vaccination (OST Clients) all 3 Doses and Monitoring. Any Hep B positive client is referred to tertiary facilities. Hepatitis C Virus We do Hep C Virus Testing (OST Clients). Hep B reactive clients are initiated on treatment. 12 Weeks VL is done to verify if client is treated. If not, client is re-initiated and put on Daily Observed Therapy (DOT).

OUR HOMELESS SHELTER

On June 12th, the Inkunzi Isematholeni Foundation, with the support of uMsunduzi Municipality, launched a dedicated Homeless Shelter aimed specifically at providing a safe space for women in need. This initiative is part of a gradual strategy to eventually open additional wings of the shelter to accommodate male beneficiaries as well, but given the heightened vulnerability of our female cohort, we prioritized their needs first. Currently, the shelter is home to 30 female residents who have undergone a comprehensive screening process for health issues, including Hepatitis C (HCV), Hepatitis B (HBV), Tuberculosis (TB), Sexually Transmitted Infections (STIs), as well as breast exams, Pap smears, and family planning services. All residents have also been initiated on Opioid Substitution Therapy (OST). In addition to health services, beneficiaries receive ongoing intensive psycho-social support in collaboration with the Department of Social Development (DSD). We ensure that all residents receive three nutritious meals each day to support their well-being. To promote physical health and community engagement, we offer sports activities such as netball and physical training sessions. Additionally, we organize movie nights to provide entertainment and foster a sense of community among the residents. This shelter represents a meaningful step towards supporting vulnerable women in our community, and we are committed to expanding our services as resources become available.
